Characterization of Mega-Antrostomies performed at Hospital del Salvador between July 2017 and June 2023

Abstract

Introduction: Sinus disease is a common reason for otolaryngology consultation. The Me- ga-Antrostomy is a surgical procedure that provides wide access to the maxillary sinus and facilitates postoperative topical therapy and can be indicated as primary or revision surgery depending on disease severity and patient diagnosis. Objectives: To describe the clinical and epidemiological characteristics of patients undergoing Mega-Antrostomy at Hospital del Salvador between July 2017 and June 2023. Materials and Methods: Retrospective descriptive observational study. Patients were identified through review of surgical proto­cols associated with the general “Antrostomy” code, manually filtering those documenting Mega-Antrostomy. Demographic variables, diagnosis, type of surgery (primary or revision), and postoperative disposition were recorded. Data were analyzed using STATA. Results: Of 421 Antrostomies reviewed, 23% corresponded to Mega-Antrostomies. The mean age was 49.7 years, with a balanced sex distribution. The most frequent diagnosis was chronic rhinosinusitis with polyps (64,9%). Primary surgeries accounted for 73,2%, and revision surgeries for 26,8%, with a higher proportion of secondary procedures in women. Most patients (53,6%) were managed on an outpatient basis; recorded hospitalizations were re­lated to non-otolaryngological comorbidities. Conclusions: Approximately one quarter of the Antrostomies corresponded to Mega-Antrostomy. Although many were performed as primary surgeries, this approach remains uncommon and selective, predominating in pa­tients with chronic rhinosinusitis with nasal polyps and in secondary surgeries in women, consistent with previous reports. Future research is needed to optimize the selection of the surgical approach according to patient profile.
