Revision stapedostomy: 10-year experience at the Clinical Hospital University of Chile

Abstract

Introduction: The available evidence indicates that revision stapedotomies are more challenging to perform, and present inferior audiometric outcomes compared to primary surgery, with success rates ranging between 40% and 80%. Objective: To perform a descriptive analysis of revision stapedotomies carried out at the Clinical Hospital of the University of Chile (CHUCH) during the period 2013-2023. Material and Methods: A retrospective review of revision stapedotomy surgeries performed at CHUCH from 2013 to July 2023. Results: During the described period, a total of 37 revision surgeries were performed, of which 34 were included in the study. 65% were performed on female patients, with an average age of 50 years. 97% of the patients underwent surgery due to the persistence of air-bone gap. The main intraoperative findings were the presence of adhesions in the tympanic cavity, erosion or fracture of the long process of the incus, and malposition of the prosthesis. Postoperative air-bone gap closure (defined as < 15 dB) was achieved in 52.9% of the patients. Conclusion: Revision stapedotomies present worse auditory outcomes compared to primary surgeries.
